--output support for gpgsm

Hello,

I have been working on an Emacs interface to gpgsm and I found that
--output is indispensable because Emacs can't handle separate file
descripters for input.

While I know that this is in the TODO list, I see no difficulty in
implementing it, so I wrote the patch attached below.
