To do this you can put a magic value into the PINENTRY_USER_DATA envvar
and let your wrapper divert to your Pinentry if that magic value is
detected.  If really required we can also add a new envvar for that
purpose but that will only work for future gpg versions (gpg uses a
static list of envvars to be forwarded to gpg-agent).
