sorry, there does not need to be two layers of OCTET string at all, *unless* we emit indefinite-form BER encoding, which is not recommended.  I misunderstood Ryan's comment, which he [clarified here](https://mailarchive.ietf.org/arch/msg/spasm/bOv69FZzykRJZSlTvUTZWGF_MJ0/).
