[issue80] certmanager: certificate wizard should allow clicking "generate certificate" only once

The "generate certificate" button in the certificate wizard can be clicked
multiple times.  After one successful execution that button should not be
clickable anymore.  Users are confused by this.  One case is that they click
once, enter their passphrase several times and thus create he key successfully.
 Then the click once more, perhaps accidentally, the passphrase dialog comes up,
they realise that they already did this and cancel.  The key generation then
fails instead of merely being aborted.  The preciously created keypair is
effectively lost.

It might be even better to integrate the keypair generation with the "next"
button action.
