[issue23] certmanager: no "generate certificate" button in certificate wizard

The wizard to generate a keypair and a certificate request used to have a button
(incorrectly called "generate certificate") on the "Decentralized Certificate
Generation") page that starts the actual key generation. Only when the key
generation was successful the "next" button was activated.  That generate button
is gone and there's no way to actually create the keypair or the certificate
request.
