Please see the bug report below.  I can reproduce this on a KDE desktop.  It 
seems to be limited to the Qt variant; the GTK variants work.  In fact, the 
Qt variant seems to ignore the "no-grab" setting, or at least interprets it 
inconsistently with the GTK variants.

i'm using pinentry-qt from kmail, in a gnome desktop, though it's
probably a general problem.  here's the scenario:

i'm in my mail program, and while navigating through messages ipass
over a message encrypted to me.  gnupg-agent starts the pinentry
program, which pops up to recieve my passphrase.  however, if the focus
goes back to another window (which it might do if i tried to navigate
to another message before realizing what happened), i can not alt+tab
through the window list to get back to the pinentry program.  it shows
up in the "task bar", and i can click there to re-focus on the pinentry
program, but that seems to be a little buggy...
