Apparently (0 0 0 and 0 0 0 and 0 and triple [none]), gpg was unable
to get information from scdaemon.  It looks like USB failure.

Unfortunately, important information is missing.  Full output of gpg
--card-status should be available, which includes information about
method of accessing the token (PC/SC or internal CCID driver).
