I have done it this way

    NEED_PASSPHRASE_PIN <card_type> <chvno>
        Issued whenever a PIN is requested to unlock a card.

I have not yet done this because BAD_PASSPHRASE takes a keyid and the
PIN verification may get hidden in sequence of other commands.  Not
sure what to do here.  I need to investigate a couple of other things
first, like what to do with readers equipped with a pinpad.
